How to prepare and register for the NDIS

  1. Use the My Access Checker or call the NDIA on 1800 800 110 to ask if you or your family member is eligible for the NDIS and what you can do prepare. You can find this My Access Checklist on the NDIS website. Visit: http://www.ndis.gov.au/ndis-access-checklist
  2. Contact us on (07) 5524 6538 to arrange an appointment time to complete your pre planning booklet together, or alternatively you can complete this on your own. See HERE.
  3. Meet with a support coordinator or planner to discuss goals and go through pre planning booklet together in more depth if necessary
  4. Arrange for diagnosis if you don’t already have one
  5. Start gathering all the paperwork you might need for the NDIS planning session. This includes forms, letters, reports, health assessments, individualised service plans, school-teacher feedback and any information that will give evidence of your disability or of the disability of the person you care for.
